Making an Appointment

Power of Attorney

For various reasons, there are times when you may not be able to take your child to the dentist in person. In order to enable another person to accompany your child to the University Clinic of Dentistry Vienna, a legal letter of authorization must be provided for that person.

In case of need, please complete the form completely. Furthermore, please notify which matters can be dealt with by the other person. The person accompanying your child must be of legal age und must bring an official photo ID (e.g. passport, identify card or driving license) to every appointment.

If you have any further questions about having other people accompany your child to the treatment appointment, please contact the respective department of the University Clinic of Dentistry Vienna.
